| MANDATORY ORIENTATION on Thurs, 4/27/17, 5-9pm in Race, 4044.
Due to blood and body fluid exposure during skills lab, and because of the health requirements of the healthcare facilities in which students are placed for their externships, all students must comply with all of the following before they are given an add code to register for the class:
1) Negative urine drug screen (done by CastleBranch - can take up to 4 weeks to process once submitted).
2) Negative background check (done by CastleBranch - can take up to 4 weeks to process once submitted).
3) Clear health evaluation form (use 4 page SRJC form)
4) 2 Step PPD (Tuberculosis skin test). The 1st TB skin test is placed and read, then the 2nd test is placed and read from a week to a month later. (Do not have the MMR immunizations immediately before this test.)
5) Positive hepatitis B titer OR a complete hepatitis B series with positive titer after last dose (takes 6 months to complete). First 2 doses must be complete in order to receive the add code, and the 3rd dose must be complete before the student will be allowed to begin the onsite externship.
6) Positive varicella titer OR complete varicella series.
7) Complete series of Tdap vaccines (if given more than 10 years ago, booster must have been completed within the last 10 years).
8) Positive titer for measles, mumps and rubella (MMR) OR a complete series of MMR vaccines.
9) Flu vaccine within the last 12 months. Declinations are not accepted unless there is a documented medical contraindication to the flu shot.
10) Pay a skills lab supply fee to pay for all training related phlebotomy supplies that are necessary for certification and completion of the class. This fee is $400.
In order to get the drug screen and background check done, as well as submit all health documentation, students will be required to purchase a package code from CastleBranch. This package costs $103.75 and is not optional. Go to https://phlebotomy.santarosa.edu/welcome-srjc-phlebotomy-program for ordering information.
