Community health workers and others serving vulnerable populations will benefit from the acquisition of knowledge and skills to assist families and individuals in preventing dental disease, accessing dental services and developing individual health promotion plans. This course is aimed primarily at SRJC students enrolled in the Community Health Worker (CHW) program as an elective to complete the certificate program, and members of the community who are employed as health promotors, community health workers or care coordinators.

2. Identify evidence-based dental disease prevention and treatment strategies for children and
adults.

2. Describe and demonstrate effective delivery of age-appropriate patient education topics.
3. Demonstrate effective patient counseling skills including motivational interviewing,
strength-based health behavior planning and conflict resolution.
4. Explain the role of the Community Dental Health Worker and key tasks in dental disease
prevention, outreach, education, disease management and referrals.
5. Describe Health Insurance Portability & Accountability Act (HIPAA), the Community Health Worker Code of Ethics and Mandated Reporter law.

1. Write a dental health education plan for each of the following patients, minimum 250 words
for each plan.
A. parent of toddler with incipient caries
B. 25 year old who smokes tobacco
C. 62 year old with diabetes II
2. Based on a scenario presented in class, develop a detailed behavior change plan to address a
patient's self-selected goals. Include goals, timeline and follow-up activities (minimum 750
words)
3. In small groups perform 2-3 role-plays to demonstrate motivational interviewing skills in
response to instructor-written scenarios
4. Final exam
