Advanced principles of periodontology for the dental hygienist, including the role of the dental hygienist in identifying appropriate surgical modalities for treatment of periodontal diseases and   providing  post surgical specialized periodontal maintenance care for dentate and  dental implant patients. Study of advanced principles of clinical dental hygiene practice; including identifying and support therapies for the various periodontal surgical modalities: resective, regeneration, prep-restorative and implant placement procedures. Differentiation and treatment of various periodontal emergencies will be discussed. Treatment for patients with specific medical modifying conditions and systemic co-risk factors will be included.

COURSE CONTENT

Outcomes and Objectives:
At the conclusion of this course, the student should be able to:

Upon completion of this course, the student will be able to:

1. Identify advantages and disadvantages of surgical therapy

2. Discuss elements of case selection for surgical candidates using intake interview, health history, and clinical       evaluation.

3. Demonstrate the ability to recognize normal osseous anatomy on a dental periapical or panorex radiographic       image and relate the suggestive appearance of bone loss in vertical defects, interproximal craters, and       furcations to the signs of disease found in clinical patients.

4. Discusss how radiographic images can be used as a screening tool to discuss implant indications and        contraindications with a patient.

5. Describe the principles and techniques of periodontal surgery.

6. Describe surgical closure and post operative care.

7. Conduct patient assessment though interview and laboratory results.

8. Discuss the correlation of periodontal infection as a risk factor to various diseases.

9. Evaluate patient's medication regimens in regards to periodontal condition and therapy outcome.

10. Describe the management of periodontal emergencies.

11. Compare and contrast the indications, contraindications, and techniques for placement of dental implants.

12. Describe the role of the dental hygienist in implant therapy.

13. Describe the impact of occlusion on the progression of periodontal destruction.

14. Explain the signs, symptoms, and the importance of  both short- and long-term treatment.

15. Describe the new technologies, drug therapies, and treatment used in periodontal care.

A. Introduction to surgical intervention in periodontal therapy

B. Principles of periodontal surgery

      1. pocket reduction or elimination

      2. excisional and incisional

      3. osteoectomy, osteoplasty, regeneration

      4. pharmacological agents used for infection control, pain relief

      5. adjunctive restorative surgical procedures

      6. predictability of surgical therapy relative to case selection

C. Surgical closure and post-operative care

      1. suture material types, recommended applications

      2. periodontal dressing materials

      3. post-operative instructions/medicaments

      4. post-operative plaque removal techniques

D. Periodontal medicine

      1. systemic risk factors: implications for prognosis and treatment

      2. periodontal infection as a risk factor in systemic disease

      3. patient's prescriptions and over the counter medications

E. Periodontal emergencies

      1. chronic periodontal abscess

      2. infected pulpitis and fractured roots, acute periodontal abscess, periocoronitis

      3. acute vesiculo-erosive diseases effecting the gingiva and oral mucosa

F. Dental implants

      1. history of the use of implants in dentistry

      2. osseointegration

      3. soft tissue interface

      4. complexity of implant restoration reconstruction

      5. predictaility of dental implants, case selection, prognosis.

      6. role of dental hygienist in implant therapy including peri-implantitis and its treatment

G. Temporomandibular joint (TMJ)

      1. occlusion plays a role in the etiology and progression of periodontal destruction.

      2. symptoms and treatment of TMJ dysfunction

H. New technologies

      1. dental lasers

      2. Non-steroidal antinflammatory drugs (NSAIDS) and tetracycline products

      3. anaerobic antimicrobial testing and therapy

      4. genetic testing

Assignments:

1. Reading assignments (3 chapters per week)

3. Semester-long case study/research paper: completion of clinical case study begun in DH79 and DH71D.

   a. Document progress and nonsurgical treatment results.

    b. Outline a post surgical treatment plan including projected outcomes.

   c. Present case study.
