Catalog Description:

This is one part of advanced level theory for the part time vocational nursing program. Concepts learned in prerequisite theory are integrated into the study of nursing care of severely acute and chronic illness patients in hospitals, ambulatory care, and skilled nursing levels. Health promotion and maintenance are emphasized. Health problems common in the elderly and legal/ethical dilemmas arising in their care are explored. Concepts fundamental to the leadership and delegation roles in the skilled nursing settings are presented.

Description:

Advanced level VN course for part time vocational nursing program which builds on previous theory courses. Focus is on the care of severely acute and chronic illness patients. Theory includes leadership concepts, care of special needs of geronotolgy clients, health promotion and client teaching that may be applied to a variety of settings.

COURSE CONTENT

Outcomes and Objectives:
At the conclusion of this course, the student should be able to:

Upon completion of the course students will be able to:

1. Apply concepts learned in prerequisite theory for the analysis and care

of needs of patients with unstable acute illness & severe chronic illness.

2. Apply Orem's Self-care Deficit Theory to clinical scenarios involving

patients with multiple self-care deficits.

3. Apply adult development concepts to the design of nursing systems for

older adults.

4. Collaborate with other vocational nursing students in solving problems

related to hypothetical clinical scenarios where the VN assumes a leader-

ship role in skilled nursing settings.

5. Compare and contrast effects of pathophysiological processes in older

adults versus younger adults and children.

6. Predict the effects of medications on older adults, including the

effects on functional status.

7. Describe common clinical syndromes encountered in older adults,

including typical nursing, medical and allied health care & therapies

such as in:

a. Degenerative neurologic and muscular disorders.

b. Mental illness, confusion, and delirium states.

c. Electrolyte imbalances as a result of multiple body system effects and

pharmacologic and other interventions.

d. Wound and incontinence diagnosing, treatment and nursing care.

e. Immune system disorders of complex and often terminal outcomes.

8. Formulate plans of care for patients in hypothetical scenarios with a

focus on diagnostics, labs, and monitoring of responses to treatment.

9. Design brief tutorials for patients with chronic illness.

10. Resolve hypothetical ethical dilemmas related to end of life issues.

11. Compare and contrast strategies and techniques employed in the care of

younger adults and children with the elderly.

12. Prepare and present a case study to peers with a thorough overview of

pathophysiology, nursing diagnoses, interventions and patient outcomes.

Topics and Scope

Topics of course include:

1. Mental Health Nursing

  a. Assessment, intervention and evaluation of common disorders.

  b. Monitoring of psychotropic and anti-seizure medications.

2. Leadership Role in the VN scope of practice in the skilled nursing

setting.

3. Care of the gerontological patient in acute, ambulatory care and the

skilled nursing settings including:

  a. Changes of aging in the normal physiologic processes

  b. Losses of aging, sexuality concerns, and special considerations with

pharmacologic drugs and fluid balance.

  c. Overall expanded focus of care of the gerontology client.

4. Intershift reports in acute and skilled nursing settings.

  a. Verbal skills for phone and direct physician orders and reporting.

  b. Written skills for transcribing of orders and documenting care.

5. Common chronic illness problems related to degenerative disorders.

  a. Nursing care of patients with conditions (i.e. Multiple sclerosis,

     Myasthenia gravis, Amyotrophic lateral sclerosis).

  b. Treatment plans including medications, teaching, rehab exercises.

6. Advanced monitoring of fluid and electrolyte status.

7. Chronic illness care related to repetitive injuries and rehab needs.

8. Advanced immunology theory including:

 a. AIDS: the current trends and treatments.

 b. Hypersensitivity: allergy categories and treatment options.

9. Common gerontologic and chronic illness problems related to skin

breakdown and complex wound care, incontinence categories and options of

treatment plan.

10. Review of cardiovascular disorders with focus on cardiogenic shock.

heart failure, pacemakers and current trends in cardiology.

11. Peripheral vascular disorders and relationship to culture and aging

with common drug and treatment plans.

12. Common pulmonary problems with review of pneumonia, asthma, and more

advanced theory related to ARDS treatment and cancer of head/neck leading

to tracheostomy.

13. Critical thinking and priority setting related to death and dying with

review of advance directives and process for meeting patient spiritual

needs.

14. Quick data collection and case study review of varied clients.

Assignments:

1. Reading assignments: approximately 50-100 pages per week.

2. Written Homework: answering of 5-20 objectives per week.

3. Case studies: approximately 1-2 per week for group discussion.

4. Quizzes: 1-2 per week.

5. Exams: approximately 2-4 exams over course of semester which prepare

  the student for NCLEX-PN test consisting of multiple choice questions

  questions primarily with a combined theory and application to clinical

  scenarios.

6. Cumulative Final Exam similar to course exams.
